Brazilian Membership Consent Form
1. Validity Period
The Brazilian Membership is valid for 1 month from the date of purchase.
Unused sessions within the validity period will automatically expire and are non-refundable.
2. Reservation & Cancellation Policy
All services are available by appointment only.
Reservations must be modified or canceled at least 24 hours in advance.
Same-day cancellations or no-shows will result in a deduction of one session.
3. Non-Transferable
Memberships are non-transferable and may only be used by the purchaser.
4. Refund Policy
Refunds are available after deducting used sessions and applicable fees, and a cancellation fee may apply.
Non-Refundable Conditions
- Memberships that have passed 14 days from the purchase date
- Memberships that have been transferred to another person
- Promotional or limited-time offer memberships
Refund Request Period
- A full refund is only available within 7 days of purchase, and only if the membership has not been used at all.
